In this position, you will conduct individual and environmental assessments for individuals with disabilities in need of assistive devices and technologies to enhance their independent living.
As the Independent Living Services Coordinator, you will maintain a caseload and thoroughly document the progress of each Consumer.
This position does involve home visits, office visits, and intensive work with individuals of all ages with disabilities living in our communities.
In addition to maintaining a caseload, you will conduct outreach activities and provide presentations as needed to the general public.
The Independent Living Services program does purchase assistive devices and technologies after all benefits are exhausted.
A good understanding of Medicare, Medicaid and community resources is essential.
The Independent Living Services (ILS) Coordinator is responsible for facilitating the delivery of independent living (IL) services that will enable consumers to function, continue to function, or move toward functioning independently in the home, family, or community.
This position will assist Consumers in achieving successful goal outcomes that are contingent on the procurement of goods and/or services provided by an appropriate vendor source.
The Independent Living Services Coordinator is directly supervised by the Director of Programs or Center Coordinator.
Essential Tasks:
1.
Understands and subscribes to the independent living philosophy of Consumer control, peer support, self-help, self-determination, equal access, and individual and systems advocacy.
2.
Conducts intakes and determines program eligibility.
3.
Works collaboratively with Consumers to develop an Independent Living Plan (ILP) designed to achieve the Consumers' identified goals.
4.
Collects and examines all information necessary to identify, arrange, provide and/or purchase needed goods and services in order for Consumers to achieve successful goal outcomes.
5.
Works in collaboration with the ILS Purchasing Coordinator to prepare and monitor Consumer budgets, as well as to ensure the timely and cost-effective provision ofpurchased goods and services.
6.
Empowers and supports Consumers in making informed choices.
7.
Provides Consumers with one-on-one IL training as appropriate.
8.
Conducts follow-up calls and/or home visits with Consumers to assess progress toward goals, including the improvement of the Consumer's functional abilities as a result ofservices provided.
9.
Maintains full, accurate, and current documentation of all information required in each Consumer Service Record.
10.
Maintains Consumer confidentiality.

Provides Information and Referral services to the community.
13.
Networks with social service, medical, educational, and governmental agencies, as appropriate.
14.
Maintains a thorough knowledge of appropriate health and social service delivery systems, with data collection on availability and eligibility requirements.
15.
Complies with LIFE's policies and procedures.
16.
Engages in other duties as assigned.

Experience:
Bachelor's Degree in the field of Human Services, with a minimum of two (2) years' experience in working with individuals with disabilities and assisting them in navigating various service delivery systems to obtain essential resources; or, any combination of education and experience that may qualify the individual for the position.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
